Required Documents

• Valid passport with 6+ months validity

  Passport must be valid for at least 6 months beyond your intended period of stay in the United States. If multiple people are included in one passport, each person requiring a visa must submit a separate application.

• Completed visa application form

  Form DS-160 must be completed online. You must answer EVERY question on the application form. If the answer is 'none', write 'none' instead of leaving it blank. Incomplete or incorrect forms will be returned and require scheduling a new interview appointment.

• Completed DS-157 and DS-158 forms

  Additional required forms that must be completed along with the DS-160 application. All questions must be answered completely.

• Passport photograph

• Flight booking confirmation

  Round-trip flight itinerary showing confirmed bookings for your intended travel dates to and from the United States.

• Hotel confirmation letter

• Bank statement

• Employment verification letter

  Printed confirmation page from the completed online DS-160 form. This is required for the visa interview appointment.
